ACCEPTING NEW PATIENTS

In order to become a new patient you will need to fill out the NEW PATIENT INTAKE FORM before calling the office to schedule.

Once the intake form is filled out please call the office to schedule your new patient appt. Thank you

Preparation for your appointment:

• Complete the intake form below

• We are located in The Women’s Center at Alta View Hospital in Building #4, Suite #235. Parking might be limited, allow an extra 10 minute for parking. Please click on the parking map link to see where we are located: PATIENT PARKING MAP LINK

• If your more then 10 minutes LATE to your appointment you will be asked to reschedule.

• Please allow 24-48 hrs. notice for all cancellations.

• Our office PHONE hours are Monday- Thursday 9 am to 3:30 pm

• YOU MAY HAVE ONE ADULT TO ACCOMPANY YOU TO YOUR VISIT, NO CHILDREN ARE ALLOWED

Insurances:

Please fill out the above patient forms. Once the forms are filled out we will then verify your insurance with our billing company. Here are some of the insurances we do accept. PLEASE keep in mind we are still in the middle of credentialing with more insurance companies. Tricare West and a few others we are still waiting to get approved.
