ACCEPTING NEW PATIENTS

In order to become a NEW PATIENT you will need to fill out the NEW PATIENT INTAKE FORM before calling the office to schedule.

Preparation for your appointment:

• NO CHILDREN ARE ALLOWED

• Please make sure to set up your portal and have all the patient forms complete before your appointment. If your link has expired, please call the front office and request a new link.

• If your more than 5 minutes LATE to your appointment you will be asked to reschedule.

• Please allow 24-48 hrs. notice for all cancellations.

• Our office PHONE hours are Monday- Thursday 9 am to 3:30 pm

Please fill out the above patient forms. Once the forms are filled out we will then verify your insurance with our billing company. If you dont see your insurance listed below please call your insurance to see if Miquelle Smith is a provider.
